New SPHM App and Algorithms from VHA

SPHM algorithms in 2014 in order to include them in a new SPHM app. Changes to the algorithms will be discussed, and the app will be introduced. The app will be available to the public on the VA App Store and is also expected to be released through Apple and Android app stores.
